Consent forms

Each college used their own consent form for students over 18. All consent forms are identical except for which logos are used, and are available for download.

• Anniesland College consent form
• Lancaster and Morecambe College consent form
• Perth College consent form
• Preston College consent form

A tailored version was also used for students who were under 18. A sample, used by Lancaster and Morecambe College, is attached. Other colleges can download this and add their own logos if desired.
