Dr Faye Tucker and University Hospitals of Morecambe Bay Trust (UHMBT) Transitions Coordinator, Val Baxter will discuss their collaborative work on developing a community of practise for NHS colleagues to support the needs of young people transitioning through to adult healthcare services.
